The replacement UV lamp for an Aqua Zonic Universal UV clarifier. Treat it as a consumable rather than a part: UV output falls away steadily and is well down long before the lamp stops glowing, so a clarifier that has quietly stopped clearing green water usually has a lamp that still lights.

Match the model number on the unit and the wattage. The model number is printed on the housing. Going by physical length alone is how people end up with a lamp that fits the sleeve but not the ballast.
Replace it annually whether or not it has failed, and wipe the quartz sleeve at the same time. A film of scale on the quartz blocks UV just as effectively as a dead lamp, and is the more common problem of the two.
Safety
Never look at a lit UV-C lamp outside its housing. It will damage your eyes.
