It is a consumable, not a one-off purchase: Fluval says replace every four weeks, and after that the insert is doing nothing but occupying a basket slot.
Each pack holds three inserts, so it covers roughly three months of changes on a single-carbon filter. The equivalent carbon-plus-Clearmax media is the AquaClear Clear Carb Insert.
