On an eccentric tip syringe the nozzle sits off to one side rather than in the middle. That small change lets you lay the needle almost flat against the skin, which is what you want for shallow work close to the surface and for anyone drawing from an ampoule. For straight intramuscular injections a centre tip is just as good, so this is a preference rather than a requirement. Supplied without needles.
